I George Kilb, aged 39 Years, occupation Physician, do declare on oath that my personal description is: Color white, complexion dark, height 6 Feet 2 Inches, weight 220 pounds, color of hair brown, color of eyes blue, other visible distinctive marks none.
I was born in Kristiansand Norway on the 11 day of March, anno Domini 1888; I now reside at 65 E. 53rd St., New York City, N.Y.
I emigrated to the United States of America from Oslo Norway on the vessel Frederik VIII; my last foreign residence was Kristiansand Norway; I am married; the name of my wife is Signe; she was born at Norway and now resides at with me.
It is my bona fide intention to renounce forever all allegiance and fidelity to any foreign
prince, potentate, state, or sovereignty, and particularly to Haakon VII King of Norway of whom I am now a subject; I arrived at the port of New York in the State of NY, on or about the 30 day of November, anno Domini 1925; I am not an anarchist; I am not a
polygamist nor a believer in the practice of polygamy; and it is my intention in good faith to become a citizen of the United States of America and to permanently reside therein: SO HELP ME GOD.
